Why did I receive a COVID-19 survey from the SCCS?

We have sent this new survey to SCCS participants who have provided an email to the SCCS. This new survey will help us understand the health of SCCS participants during and after the coronavirus (COVID-19) pandemic. All SCCS participants, whether they have been ill with COVID-19 or not, are invited to complete the survey.

How do I update my email address with the SCCS?

You can update your contact information by calling us at 1 (800) 734-5057 or by sending us a message through our contact form.

Can I complete the SCCS COVID-19 survey on paper or by phone?

If you prefer to complete the survey by phone, please give us a call at 1 (800) 734-5057 Monday-Friday between 9 am and 5 pm Eastern time. Currently, we do not have a paper version of the survey available.
